Membership:
A 6-Month Membership is required to be established as a patient with Wave
Concierge. This membership fee is not covered by insurance, but rather is patient choice.
Insurance:
All in-office/facility visits and/or visits to patients residence will file to the patient’s
insurance and will process according to the patient’s policy, benefits, and terms. *With the
exception of the initial 60 minute and subsequent 60 minute annual comprehensive
consultation and physical; the Doctor’s services for those visits WILL NOT bill to insurance.
However any other services provided at those comprehensive visits (vaccines, EKG, MRI,
X-Rays, Ultrasounds, etc) WILL bill to patient insurance.* Any co-pay or patient portion
remaining after insurance adjustment will be the ‐ responsibility of the patient.
Starting Your Annual Membership:
Your 6-Month Membership at Wave Concierge begins once
the Membership Agreement is signed and completed by the patient, and the initial payment is
processed.
Wave Executive Membership Inclusive Benefits and Services

persons who sign up as Members:
• As a member you’ll see your assigned concierge doctor—not someone else. If your
concierge doctor is away, telemedicine or NP visit is available.

• 60 minute Comprehensive Consultation and Physical
• 1 hour Annual Physical Examination
• 30 minutes of personal care for each appointment

• Four complimentary IV vitamin infusions per year
• Four complimentary aesthetic facials per year
