Wave Concierge Policies

These policies apply to the Wave Concierge website and membership purchases. Your signed Membership Agreement identifies your plan, price, benefits, membership period, and any terms specific to your enrollment. If a description on this page differs from your signed agreement, your signed agreement controls your membership.

Shipping Policy

Wave Concierge memberships are services; no physical product is shipped when you purchase a membership. After enrollment, our team will coordinate the agreement, payment information, and any information needed to begin your membership. If a physical item is offered separately, its delivery details will be provided at the time of that purchase.

Cancellation Policy

You may end your participation by giving 30 days’ prior written notice as required by your Membership Agreement. Cancellation does not automatically entitle you to a refund for an unused portion of your chosen membership period.

Membership fees are billed according to the plan and payment schedule in your agreement. Failure to pay a renewal fee may result in termination of your membership. Wave Concierge may also terminate a membership with 30 days’ written notice and a prorated refund, as provided in the agreement.

Refund Policy

Refunds are generally unavailable once membership services have been rendered. These services may begin before the first appointment and can include obtaining and reviewing records, coordinating care, and preparing for the initial consultation.

In rare and exceptional circumstances, a refund may be considered. If approved, the refund is prorated for the unused portion of the membership, less a cancellation fee equal to 25% of the total membership price, as stated in the Membership Agreement.

If Wave Concierge terminates your agreement with 30 days’ written notice, the agreement provides for a prorated refund. For a billing or refund question, call or text (561) 800-3188.

Terms of Service

Membership and services. Wave Concierge offers membership administration, enhanced access, scheduling, communication, and care coordination in connection with participating medical providers. Your particular benefits are those stated in your signed Membership Agreement. Enrollment requires completion and acceptance of the agreement and applicable intake documents.

Medical care and insurance. A membership fee is separate from health insurance and does not replace it. Except for items expressly included in your plan, medical visits, tests, procedures, and other healthcare services may be billed separately by the medical provider and may be subject to your insurance terms, copayments, coinsurance, and deductibles. Medical decisions and treatment are made by licensed healthcare professionals.

Appointments and availability. Appointments and specific services depend on medical appropriateness, provider availability, and the terms of your plan. New patient visits are by appointment. Website descriptions of access and scheduling describe membership features; availability of a particular appointment, test, hospital admission, or outcome cannot be guaranteed in every circumstance.

Payment and renewal. You agree to provide accurate billing information and keep your authorized payment method current. Your membership fee, billing interval, chosen duration, and any payment commitments are shown in your agreement. You remain responsible for charges incurred under that agreement and for separately billed healthcare costs.

Website use. Information on this website is general information and is not a diagnosis or a substitute for advice from a licensed clinician who has evaluated you. Do not use a website form or ordinary email for an emergency. Call 911 if you believe you have a medical emergency.

Changes and governing terms. We may update this website page from time to time. Changes to a signed Membership Agreement require the written agreement of the parties as stated in that agreement. The agreement is governed by Florida law and contains additional terms concerning notices, amendments, and disputes.

Privacy Policy

Information we collect. When you contact us, use the website, request a consultation, or enroll, we may collect information you provide, such as your name, contact details, appointment requests, membership and billing information, insurance information, and information needed to coordinate your care. The website may also collect technical information needed for its operation, such as browser and device information.

How we use information. We use information to respond to inquiries, administer memberships and payments, arrange appointments, coordinate with participating providers, maintain and improve the website, and comply with applicable obligations. We may share information with providers and service companies as needed for those purposes, subject to applicable privacy requirements.

Medical information. Your medical provider’s Notice of Privacy Practices explains how protected health information may be used and disclosed and describes patient rights. Please review that notice together with this website policy. Where a signed authorization is required for obtaining or disclosing records, the authorization governs that exchange.

Payments and service providers. Payment and website service providers may process information on our behalf to operate the site, manage memberships, and complete transactions.

Communications. Ordinary email may not be a secure way to send sensitive health information and should not be used for urgent or time-sensitive requests. Contact the office by phone if a matter needs prompt attention.

Your choices and questions. You can contact us to correct membership contact information or ask how your information is handled. Requests concerning medical records and rights under your medical provider’s Notice of Privacy Practices should be directed to that provider.
